ALL THE INFORMATION NEEDED IN ORDER TO APPLY FOR A PERMIT IS INCLUDED BELOW.  PLEASE PRINT THE NECESSARY FORMS FROM YOUR COMPUTER AND SUBMIT THE COMPLETED FORMS TO THE VILLAGE OFFICE.  THANK YOU.

Guidelines for Obtaining a Building Permit
(Checklist of what’s needed)
Amendment to Application for Building Permit
(Required for changing a building permit)
Affidavit of Final Cost of Construction/Application for Certificate of Occupancy (Statement of final cost needed at the end of construction to close out a building permit)
Nassau County Board of Assessors Building Permit
(Required by Nassau County at the time of filing the building permit)
SEQR Short Environmental Assessment Form
(Required by Nassau County at the time of filing the building permit)
Affidavit of Exemption of Workmen’s Compensation Insurance
(Needed when homeowner performs all the work)
Affidavit of Homeowner as Self Contractor
(Needed when homeowner performs all the work)

Application for a Variance or Conditional Use :
(Needed after denial of building permit application)

(List of items needed for variance application)
SEQR Short Environmental Assessment Form
(attach the same SEQR form that was filed with the building permit application)
Consent Form
(Not essential to application; only needed if homeowner wishes to obtain written consent from neighbor(s))
Properties Within Radius of 200 Feet
(Listing of properties of neighbors to be notified about application for variance)
Notice to Owners of Adjacent properties
(To be sent to neighbors whose properties are within 200 feet of applicant’s property)
Affidavit of Mailing of Notice
(Applicant’s statement that neighbors were notified)
Instructions Regarding Conflict of Interest Affidavit
(Directions about how to fill out conflict of interest affidavit)
Affidavit of Disclosure
(Applicants have to disclose relationships with municipal officials and related parties)

Application for Tree Removal Permit
(Needed prior to removal of any tree)

Road Opening Permit Application (Public Works Dept)
(Needed by homeowner for gas connection/electrical or plumbing work)

Pavement Specifications for Road Openings
(Detailed instructions for work)

Application for Permit for Dumpster, Fence, Road Opening, Curb Cut
(Needed prior to installing fence, placing dumpster or making new curb cut

Addendum to Fence Permit-Remedy of Violation
(Agreement to move fence if placed improperly)

Application for the Erection and Maintenance of a Display Sign
(Needed for all advertising structures and signs)
